When a doctor is to work in a rural remote area
you need to be a jack and jill of all trades
the location may be beautiful, interesting and with diverse clinical medicine
what are the benefits ?
to gain fantastic clinical experience
you may have to do a little bit of everything
How to cope with a clinical situation where you have to do a surgery while there is no anesthetist,no adequate logistics and inappropriate brand of instruments for which you are not responsible but you have to have to come out with the public criticism for the out come of your work which you have to do to save the patient.
you may see a patient in the emergency dept, review them in the ward the day after and then to see the patient in the outdoor after a week
You have minimum facilities of living but are answerable for any problem of the hospital where people think that everything is to be done by the doctor though you read that the health care is the team work
you have the advantage of having continuity of care of the patient independently and it is the rewarding part of the job
As a doctor you are more independent and have a greater clinical variety and have greater autonomy rather than the structured hierarchy based tertial care
You earn the capability of having specialist skills in OBS and GYNAE and manage deliveries or have the capacity of retrieving patients from other locations
You are exposed to public of all settings as much as even public representatives need have to face
You have to face the hierarchy of administration even public aggression and need be very much protective of saving yourself from bullies and to be vigilant al times of day and night
you have to sacrifice your social life and family life
When the patient is not healed you need have to face emergency emotions of the relatives which may lead to breaking down of glasses etc unless you know how to properly counsel the patient s relatives about the patient's condition and the options and outcome of the treatment and to explain that you are simply helping as far your knowledge and skill go but the ultimate saviour is God the almighty
If you are inquisitive you may a part of the community , be empathetic and truly responsive to them with honest intention of serving them
You may have a relaxed life style if you manage to make
But you have to remember
FURTHER CHALLENGES
You are working with limited resources but even if anybody is not appreciating you need not have to mind
You may have a sense of isolation but you have to overcome it
even if you have to work in a threatening environment you have to overcome it
Many of you are not grown up in a rural area but you need to have a smartphone to communicate to your parents , even wife and children and friends
so you have to be master of all trades and that may be the public expectations from you
But after working there you need to be an anthropologist doctor who can work in areas of turbulent hazards and remote jungles
and that will be an added experience in your life and nobody can predict where you will work in this world as this art is universal and anywhere in the world you may have to work where others may not have such experiences as you have
There is still a Jigsaw puzzle about the good public health care of Bangladesh and Prestigious International Medical journal LANCET made a comment public health is a paradox'with such a low health budget and a chaotic health system how Bangladesh could achieve such a good health index even with so limited ,resources and badly managed health system, nobody claimed who is responsible for this credit and no article is yet been published in response to this paradox comment
I think someday the answer may be found.
